Get Support with Fortitude Care

Simple, Personal and Supportive Onboarding

Getting started with Fortitude Care is simple. We aim to make the referral and onboarding process clear, respectful and efficient.

Step 1: Submit a Referral

Complete the Fortitude Care Referral Form so we can understand the participant's support needs, goals, transport requirements, mobility needs and key contact details.

This helps us review service suitability and respond to short-notice support or transport requests where possible.

Step 2: We Review the Support Needs

Our team reviews the referral and may contact the participant, family, guardian or support coordinator to clarify:

  • Support goals
  • Preferred days and times
  • Transport needs
  • Mobility and wheelchair requirements
  • Health and safety considerations
  • Communication preferences
  • Staff matching preferences
  • Funding and service agreement requirements

Step 3: Complete Required Documents

Before supports commence, Fortitude Care may require:

  • Service Agreement
  • Participant Consent Form
  • Risk Assessment Form
  • Transport Risk Assessment, if transport is required
  • Support Preferences Form
  • Community Access Support Plan, if applicable
  • Any relevant health, behaviour support or emergency information

Step 4: Staff Matching and Service Setup

We aim to match participants with support workers based on their needs, goals, preferences, communication style and safety requirements.

Where requested, authorised representatives may receive shift updates or follow-up communication.

Step 5: Support Begins

Once onboarding is completed, Fortitude Care can commence supports in line with the agreed service arrangement.
